| DRESS
CODE: |
| A
neat appearance is essential to the
student's professionalism. The
Academy provides students with one
school smock. The student is
expected to keep it clean and in good
condition. Clean slacks or jeans
in good condition are required, no
shorts are allowed. Only closed-toe
shoes are permitted. |
| EXPULSION: |
| The
following matters are deemed to
be cause or expulsion from the Academy: |
- Excessive
absence
- Excessive
tardiness
- Conduct
that could endanger the health
and welfare of humans or animals
- Use
of drugs or alcohol during school
hours
- Students
who do not maintain an average
of 75% will be placed on a 2 week
probation period. If the
student fails to bring grades
up to the required average, he/she
will be terminated at the discretion
of the school director.

| ACADEMY
POLICY RELATIVE TO: |
| Leave,
absences, make-up work, tardiness
and unsatisfactory attendance: |
| If
student is terminated for any above
reasons he/she may be reinstated at
the discretion of the Academy at a
reinstatement charge of $50.00. |
| A
$25.00 fee per day will be charged
for any missed days. Fee is
payable on the following day of class. |
| Medical
absences with a letter from a doctor
will not be charged. Monday nights
and Saturdays are scheduled for make-up
hours. |
| WITHDRAWAL
FROM SCHOOL: |
| If
a student wishes to withdraw after
classes or it discontinued before
completion of the program, the sum
charged for tuition, fees and other
charges up to the last day of actual
attendance, will be as follows: |
| A
- When written notice of cancellation
is given within 6 days after date
of enrollment but prior to the first
day of class, all application registration
fees, tuition, and any other charges
shall be refunded. |
| B
- When written notice of cancellation
is given after the sixth day following
enrollment but prior to the close
of business on the student's first
day of class attendance, the school
may retain no more than the application
registration fee which may not exceed
$100.00. |
| C
- When written notice of cancellation
is given after the student's completion
of the first day of class attendance,
but prior to the student's completion
of 5% of the course of instruction,
the school may retain the application
registration fee, an amount to exceed
10% of the tuition and other instructional
charges or $300.00 whichever is less,
plus cost of any books or materials
which have been provided. |
D
- When written notice cancellation
is given when a student has completed
in excess of 5% of the course instruction,
the school may retain the application
registration fee and the cost of any
books or material which may have been
provided by the school but shall refund
a part of the tuition and other instructional
charges in accordance with whichever
of the following applies:
